No, you do not need to send another application form to change your contribution level to a registered plan.

You can log into your Shareworks account and do the changes from your dashboard. Locate the task section and you will see the Change Contributions option under the section titled You are contributing. Alternately, you can contact the Participant Experience Group and one of the representatives can do it for you over the phone.
